By completing the payment, you are confirming that you will be attending the course. No showing the day of the event will not guarantee hours completed for the course. Attendance will be taken each day of class to confirm completion of the material along with a written/practical exam. Board fees are not included in the cost of the course and are not handled by Fascia Advancement Academy. Retakes will be available online after the course.

• I, the undersigned, verify that this registration is complete, and to the best of my knowledge, all information provided is factual and true. I understand that failure to provide the needed information and required documentation could likely lead to delays in the processing of CE hours. By clicking the "submit" button below, I acknowledge that the course(s) I am registering for are continuing education courses and that upon successful completion of the course(s) I will be awarded and receive a CE certificate with the corresponding hours of awarded education. I understand that FAA is not a school and that the course(s) I am registering for are neither college courses nor are they intended for any type of transfer credits to any schools or colleges/universities and will not be accepted toward any type of college degrees.

• This General Media Release (the “Release”) is executed and effective as of the date set forth below by and between ADB Interests, LLC (together with its parents, subsidiaries, and affiliates, the “ADB Companies”) and the undersigned (“I”, “me”, “my”).

• For valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I grant to the ADB Companies, its assignees and licensees, the irrevocable right, in the ADB Companies’ sole discretion, to photograph, film, videotape, record, and/or otherwise fix or reproduce my name, image, voice, sound effects, actions, behavior, likeness, simulated likeness, biographical material and otherwise (collectively, the “Results”) and I grant to the ADB Companies, its assignees and licensees, the irrevocable right, in the ADB Companies’ sole discretion, to use and exploit the Results in whole or part, in perpetuity, in broadcast media, print media, digital/online media, and any and all other forms of media, means, methods or processes, now known or hereafter devised. I acknowledge and agree that the ADB Companies shall own all rights, title, and interest, under copyright and otherwise, in and to the Results in perpetuity for all purposes, uses, and media in the ADB Companies’ sole discretion. I waive all moral rights in connection with the Results and/or the ADB Companies’ use thereof.

• I have the legal right and capacity to enter this Release and to grant the rights granted herein. In the event of any breach of this Release by the ADB Companies, I hereby acknowledge that I shall be limited to the right to receive monetary damages only (if any) and I waive the right to seek injunctive or other equitable relief. The ADB Companies may assign this release (and/or any and all of the rights contained herein) to any third party.

• This Agreement is binding with respect to all photographs, films, videotapes, recordings, or other reproductions of my name, image, voice, sound effects, actions, behavior, likeness, simulated likeness, biographical material, and otherwise.

• This Agreement is governed by the laws of the State of Texas, without giving effect to the choice of law rules, and is subject to the internal laws of the State of Texas.

• Any claims arising hereunder shall be resolved exclusively in the State District Courts having jurisdiction in Houston, Harris County, Texas.
